Case 23: Lumps under the arm

A 59 year old man presents with a painless mass under the left arm. This was first noticed 6 weeks ago and is increasing in size.




1. What is the differential diagnosis?

Painless significantly enlarged nodes should raise the suspicion of lymphoma.

Others:

Infections : acute - local causes, systemic illnesses
bacterial - gonorrhoea
viral - IM, CMV, measels, rubella, chickenpox
Chronic - HIV, TB, syphilis, fungi

Neoplasms : primary - Hodgkin's disease, non-Hodgkin's, leukaemias
Secondary : metastasis

Sarcoidosis

Allergy


1. What questions would you ask him and what else would you examine?

All other nodal groups, abdomen - liver, spleen, ascites, PR exam
